Neuromodulation digital therapeutics pioneer Theranica announced the publication of a landmark, peer-reviewed clinical validation study in Neurology® Open Access evaluating Your Day Ahead, an integrated machine learning (ML) feature inside the Nerivio® mobile app .
The investigation, titled “ Machine Learning Assessment of Next-Day Migraine Likelihood Using Data From 53,000 App Users Living With Migraine ,” analyzed 770,473 daily diary reports from 53,065 real-world users of the FDA-cleared Nerivio wearable between January 2020 and July 2025, representing the largest prospective digital migraine dataset assembled to date.
The top-performing algorithm—a customized XGBoost classifier trained across daily diary entries, pre-treatment questionnaires, patient demographics, and local weather patterns—forecasted next-day migraine attacks at 91.2% precision, 81.0% accuracy, 80.0% sensitivity, 83.0% specificity, and an area under the curve (AUC) of 0.893.
Algorithm performance remained robust across demographic subgroups, demonstrating no statistically significant variation between female and male cohorts (p = 0.42) or between adult and adolescent patient populations (p = 0.28).
Researchers trained and evaluated seven machine learning architectures on patient diary logs, pre-treatment questionnaires, demographic data, and localized ambient weather metrics:
High Predictive Accuracy: A customized XGBoost classifier achieved 91.2% precision, an AUC of 0.893, 81.0% accuracy, 80.0% sensitivity, and 83.0% specificity.
Demographic Robustness: Model performance held without statistical degradation across biological sex (p=0.42) and age groups (p=0.28), confirming reliability across both adult and adolescent populations (Nerivio is indicated for patients aged 8 and older).
Pain Spectrum Scope: The forecasted next-day migraine likelihood accounted for mild as well as moderate and severe pain events.
