Aspirin rarely used for colorectal cancer prevention

People with Lynch syndrome have a significantly increased risk of developing colorectal cancer and other types of cancer due to a hereditary predisposition. The CAPP2 study had already shown in this high-risk group that acetylsalicylic acid can significantly reduce the long-term risk of colorectal cancer. The study examined a comparatively high daily dose of 600 milligrams. The recently published international CaPP3 study now suggests that even a lower dose of 100 milligrams daily could have a similar cancer-preventive effect while causing fewer side effects, particularly bleeding complications. The research is published in The Lancet Gastroenterology & Hepatology journal.

"Since the statistical criteria for formal equivalence between the two studies were not met in all analyses, the results should still be interpreted with some caution," says corresponding author Dr. Robert Hüneburg, a senior physician at Medical Clinic I at the UKB, who also conducts research at the University of Bonn. "However, the results of the CaPP3 study suggest that 100 mg daily could be a practical and better-tolerated option for many patients."

The survey of 539 people with Lynch syndrome from across Germany was designed before the publication of the CaPP3 results and conducted between December 2025 and February 2026. It therefore reflects the use of acetylsalicylic acid based on the scientific evidence available at that time. "Future studies must show whether the new findings on lower dosages will contribute to acetylsalicylic acid being used more frequently for cancer prevention," says Hüneburg.
